Write Lead-Ad Questions Your Team Will Actually Use
A lead form should collect information that changes what happens next. Every extra question introduces work for the person responding and for the team reviewing it. Design the form backward from the follow-up decision instead of copying a competitor's application.

The decision to make
For each proposed field, name the decision it supports. Remove questions whose answers will never affect routing, eligibility or the next conversation. Use plain choices where possible and reserve open text for information that cannot be captured reliably otherwise.
A practical workflow

Worked example
A service provider might need location and project type before assigning a callback. Asking for a favorite social platform would add little unless it affects the service offered.

Do not collect sensitive information just because a form supports a text field. Keep the request proportional to the actual service inquiry.
